Pre-certification Instructions

It is the responsibility of the referring physician’s office to obtain pre-certification for a PET/CT scan through any insurer requiring prior authorization. Pre-certification must be obtained prior to the patient’s appointment and list the correct facility/location where the patient is having their scan. To book a study, please refer to the list of locations and hours of operation for each site listed below. You may also download the Physician Order Form for each site.
